NMC Issues Draft Publication on NEXT with Information, Requests Comments; View Detailed Analysis

The National Medical Commission, the country’s highest medical regulatory body, has at last published the proposed draught regulations related to the National Exit Test (NEXT), clearly indicating that it will serve as both a licentiate exam to practice modern medicine for both Foreign Medical and Indian medicos Graduates as well as a router entrance Examination for admittance to courses Postgraduate Medical Education.

The document also includes all information about the NEXT test, including its purpose, scope, division into STEP 1 and STEP 2, how these two examinations are administered, how the NEXT scores are calculated and validated, and how they are used. It is unknown at this time whose organization will administer the test. The document states that NEXT STEP 2 will serve as a qualifying exam and that NEXT STEP 1 marks would be used for PG entry. There is no restriction on the number of trials at either exam as long as the individual has cleared both the NEXT Step 1 and NEXT Step 2 Examination within ten years of enrolling in the MBBS program. The legitimacy of the NEXT STEP 1 scores towards PG entry shall be three years.

NEXT step 1

The draught makes the following notations about the NEXT Step 1 description. 

  • A theoretical examination will be the NEXT Step I.
  • There will be 1 or more different types of multiple-choice questions in the questions.
  • The exams must be given in an online or computer-based format.
  • A body or entity designated by the council will administer this as a Centralized Common All India Examination.
  • The exam will include six exams comprising both Part 1 and Part 2 of the III and Final MBBS programs.

 Medicine and allied disciplines, Surgery and allied disciplines, Obstetrics and Gynaecology, Paediatrics, Otorhinolaryngology,    Ophthalmology, Applied aspects of all subjects covered under I MBBS and II MBBS, Applied aspects of all subjects covered under III MBBS /Final MBBS Part 1

  •  All candidates who have successfully finished the III MBBS/Final Mbbs education from a medical college that has been accredited by the Commission are qualified to take the examination.
  • Regular NEXT Step 1 Exams will be held once per year for the III MBBS (Part 2)/Final MBBS preparatory to the Mandatory Rotating Internship and University Practical Examination
  • Only applicants who failed one or more of the six papers and must retake those specific papers or subjects will be allowed to take the once-per-year NEXT Step 1 Supplementary Examination.
  • Those who fail both the NEXT Step 1 Regular and the Supplementary Examinations in a given year may only take the NEXT Step 1 Regular Examination in the year after.
  • If the applicant has completed both the NEXT Step 1 and NEXT Step 2 Examinations within ten years of enrolling in the MBBS Course, there is no limitation on the number of tries to appear in NEXT Step 1.
  • There’s no limitation to the number of times you can show up for the NEXT Step 1 Regular Exam to raise your score as long as you do so at least a year from finishing the NEXT Step 2 exam. 
  • The institutional Theory/traditional university Examinations of the III MBBS/Final MBBS Part II shall be replaced by the NEXT Step 1. Part I and Part II of the III MBBS/Final MBBS exams Unless otherwise specified by the Commission, practical/clinical examinations will continue to be undertaken traditionally.

NEXT step 2

This is what the document says about NEXT Step 2:

  •  Next Step 2 will consist of a clinical/ practical and viva voce exam encompassing seven clinical subjects/disciplines;

Medicine and allied disciplines, Surgery and allied disciplines, Obstetrics and Gynaecology, Paediatrics, Otorhinolaryngology,           Ophthalmology, Orthopaedics, and PMR (Physical Medicine and Rehabilitation)

  • The test will be objectively organized, clinical case-based, and use simulated patients and cases to assess practical and clinical skills, clinical judgment, and communication abilities anticipated of an Indian medical graduate.
  • The test must be taken in person or live, and it must be administered by the relevant state health universities or institutions by the norms and directives given by the Commission. The Commission will choose the institution or university that is permitted to perform NEXT Step 2 for the relevant colleges where there are no state health universities.
  • There will be a yearly Regular NEXT Step 2 Exam.
  • A NEXT Step 2 Supplementary Examination is only open to applicants who have ended in failure one or more (up to three) of the seven courses and are obliged to retake certain subjects. It is held once every year.
  • The NEXT Step 2 Regular Examination will only be offered to candidates who pass both the NEXT Step 2 Regular Examination and Supplementary Examination of a given year.
  • If the student has completed both the NEXT Step I and NEXT Step 2 Examinations within ten years of enrolling in the MBBS Course, there is no limitation on the number of tries to sit in NEXT Step 2.

Details about NEXT Step 1

The NEXT Step 1 will be a thorough computer-based test based on premium multiple-choice questions (MCQs), guaranteeing impartiality and adhering to nationwide transmission and execution with strong authenticity in a safe manner. The MCQ questions would cover more advanced subject areas that were related to the abilities required of an Indian medical graduate or a graduate using the modern medical system, particularly in India. 

Allocation of Items and Questions’ Knowledge Level

  • A majority of the questions must assess higher-tier comprehension, analytical abilities, and clinical problem-solving that are related to competencies to assure evaluation of the upper realms of medical education.
  • The NEXT Step 1 method aims to deter medical students from rote learning.
  • Case studies and clinical case scenarios will make up the majority of the exam.
  • The distribution of MCQ questions across all subjects will be rough as follows;

Analytical and problem-solving skills kinds 65% clinical vignettes 15/ hr of the exam, Comprehension type 25%, Recall type 10% but it’s possible to anticipate that the distribution will vary.

  • The level of understanding necessary to get skills will be roughly

 Should Know: 60%, Good to Know: 30%, 10% May Know

Papers and Subject Distribution

  • There will be six subject exams on the NEXT Step 1 test, each with its item weighting and time allotment.
  • All other MBBS courses, including Forensic Medicine and Toxicology (FMT) and Community Medicine, will incorporate practical components into the pertinent clinical subjects.
  • 10% of the items in each paper must deal with applied areas of physiology, anatomy, biochemistry, pathology, microbiology, pharmacology, and forensic medicine that are pertinent to the disciplines being assessed.
  • 10% of each paper’s components must relate to community medicine or public health applications that are pertinent to the topics being assessed.
  • The subjects in each paper are listed in Table 1 below along with the items and time allotted for each subject, along with a provision for 20% of the items distributed.

Details about NEXT Step 2

After completing an internship, there will be a thorough clinical examination or practical to assess candidates’ competencies in clinical diagnosis, patient and clinical decision-making, and the practical and communication skills required for the practise of modern medicine, particularly in India. 

Evaluation procedures

  • Each test would include the following:
  • Simulations if possible
  • Objective Structured Clinical Examination (OSCE)
  • Actual cases

There will only be two levels of grading available: “Competent or “Pass” / “Not Competent” or “Fail,” depending on the likelihood of satisfactory performance or unsatisfactory performance. 

Allocation of Subjects

NEXT Step 2 will analyze the following subjects:

  1. Medicine and allied subjects
  2. Surgery and allied subjects
  3. Obstetrics and Gynaecology
  4. Pediatrics
  5. Otorhinolaryngology
  6. Ophthalmology
  7. Orthopaedics and Physical Medicine & Rehabilitation (PMR)

Scheduled time

Upon the completion of the internship and payment to the admission procedure for the Postgraduate Courses of that year, the National Medical Commission and the relevant authorized Universities / Institutions shall determine the timing and procedures of NEXT Step 2.

Timetable for the NEXT Examination and related activities

The duration of the complete NEXT Examination, internship, and admissions procedure for broad specialty The Commission and other authorities designated by the Commission must announce postgraduate courses.

The timetables that are suggested or likely are included in the table below, but they are susceptible to change and are duly noted when they do.

If successful applicants for the Supplementary Examination are not permitted to proceed with successful candidates for the Regular Examination
